The importance of planning, the roles of both parent/guardian and student and the student’s post-secondary vision.

* Transition Planning starts at age 14 and involves creating goals for your child’s education, employment, and independent living.
* How do you and your child envision their adult life? What pathway will help get them there?
* What life skills do they still need to develop? What supports are available to families as students become adults?
